FMGE Exam 2026: The Complete Guide to Eligibility Criteria, Exam Pattern, Preparation Strategy, And The FMGE PYQs Advantage

| Updated: January 24, 2026 17:48

The Foreign Medical Graduate Examination (FMGE) is one of the most critical entry-level exams for Indian medical students who have completed their medical education in foreign countries and are keen to practice as medical professionals in India. The FMGE exam, conducted by the National Board Examinations (NBE), not only tests the students for their conceptual knowledge but also tests their knowledge of how to apply their conceptual knowledge of medicine to real-life situations. Alongside learning about the pattern of FMGE exams, it is equally important to focus on FMGE Previous Year Questions.

Understanding the FMGE Exam

The FMGE test is a screening test that is compulsory for Indian Nationals/Overseas Citizens of India (OCI) who have obtained their first medical qualification from a foreign medical institution. Clearing the FMGE test is a requirement for a candidate to be eligible to register with the National Medical Commission (NMC) and practice medicine in India.

FMGE has a reputation for moderate to very high difficulty levels, largely since it encompasses all 19 MBBS subjects in one single exam. In contrast to university-level tests, which have specific topics to cover, FMGE requires an integrated understanding of all medicine sectors.

Eligibility Criteria for FMGE

In order to appear for the FMGE examination, one needs to satisfy certain eligibility criteria. The candidate applying for the examination needs to be either an Indian national or an OCI cardholder. The medical degree needs to be recognized by the medical council of the nation to which the candidate belongs. It is also an essential criterion for the candidate to hold an Eligibility Certificate from the NMC, unless exempted under certain eligibility conditions.

Another key requirement is the completion of the primary medical qualification within the cut-off date, as mentioned in the official notification of the FMGE exam result. Without fulfilling these requirements, it is not possible to appear for the exam.

FMGE Exam Pattern and Structure

This FMGE examination is carried out through online, computer-based testing. This test consists of a total of 300 multiple-choice questions, which are categorized into two parts: Part A and Part B. Each part has 150 questions, and a scheduled break is provided in between, which is 2 hours and 30 minutes each.

There are no minus marks for wrong answers in FMGE. This encourages aspirants to answer all questions without any fear of getting marks deducted for wrong answers. But to come out successfully, one has to obtain at least 150 marks out of 300, i.e., 50%. As such, it is a qualification test. Hence, there are no ranks for aspirants. They are either successful or unsuccessful.

The questions are set in preclinical, paraclinical, and clinical disciplines with a greater weightage on clinical disciplines such as Medicine, Surgery, Obstetrics & Gynecology, Pediatrics, & Community Medicine.

Subjects Under FMGE Examination

The syllabus encompassed by FMGE is almost similar to the curriculum for MBBS in India. The Pre-Clinical disciplines comprise Anatomy, Physiology, and Biochemistry. The Para-Clinical disciplines encompass Pathology, Pharmacology, Microbiology, Forensic Medicine, and Community Medicine. The Clinical disciplines encompass Medicine and its related disciplines, Surgery and its related disciplines, Obstetrics and Gynecology, Pediatrics, Orthopedics, Psychiatry, Dermatology, Anesthesia, ENT, and Ophthalmology.

In the wake of such a huge syllabus, it becomes essential to be strategically planned instead of random.

Why FMGE Is Considered Challenging?

One major reason why FMGE is considered tough is the conceptual nature of the questions. FMGE doesn’t ask many questions based on theory. It is more oriented towards scenarios and concepts. Moreover, FMGE is also tough for candidates due to the differences in the standards and patterns of curricula in foreign medical colleges and Indian medical curricula.

Another issue is the wide-ranging curriculum, besides the pressure of having a solo qualifying exam. Since the FMGE examination is held only twice a year, if one fails, it will result in a gap in the career advancement process.

What Are FMGE PYQs?

FMGE PYQs stand for the Previous Year Questions given in the FMGE examination conducted during the last few years. These questions act as very important tools to understand the examination pattern, most likely areas of questioning, and the difficulty level.

FMGE PYQs are more than just a revision exercise. They serve as the roadmap for preparation. Several candidates have found that there is a substantial number of questions that get repeated directly or indirectly, with minor changes to the clinical scenario or the options.

Relevance of FMGE PYQs in Preparation

FMGE PyQs also help candidates to identify topics that yield high marks. Analyzing past questions will help candidates understand what is being consistently tested, such as the pharmacology of commonly used drugs, the pathology of common diseases, health programs of our country, obstetric complications, and emergency cases of internal medicine.

Preparation of FMGE PYQs will also help in improving the management of time as well as decision-making. The reasons are that a total of 300 questions are to be completed. Hence, speed as well as accuracy are of utmost importance.

Effective Use of FMGE PYQs

It is not adequate to only solve FMGE PYQs. The methodology is also important. Every candidate needs to first complete their fundamental reading in each subject to solve PYQs. After setting up this base, it is essential to work on PYQs in a topic-based manner instead of in an unsystematic fashion.

It is also necessary to analyze the reasons behind each problem being solved through a particular option being right and others being wrong after finishing each problem. Use a separate notebook for concepts based on PYQ for ease of revision before exams.

Regular revisions of FMGE PYQs, particularly in the last 5 to 10 years, are very effective in developing confidence. Most toppers will agree upon recommending a minimum of three revisions of PYQs prior to sitting for the examination.

Preparation Strategy for FMGE with PYQs

An ideal preparation plan for FMGE would be a mixture of conceptual understanding, PYQ solving, and mock tests. Clinical subjects would require greater emphasis as they contribute maximum marks and dominate the PYQs.

Study Strategies to Avoid During FMGE Preparation

The first common blunder made by the candidate is overlooking the PYQs during initial preparations. The next one is the emphasis on memory instead of understanding the concepts. The FMGE questions have a habit of distorting the concepts.

Overloading of resources will also confuse. It is better to restrict resources to the essentials and regularly work on FMGE PYQs.

Final Thoughts:

The FMGE exam does not require merely memorization but a test for comprehensive knowledge and understanding. With proper strategy, hard effort, and consistent practice with FMGE PYQs, cracking the exam can be achieved.
